IT’S THE LAW

Effective April 14th, 2003, the Fund can no longer provide you with information about your personal files over the telephone without first being able to  establish that you are who you say you are.

Since 2002, and every three years, The Fund’s Newsletters list important information regarding access to your Fund information.  One of the provisions of the Health Insurance Portability and Accountability Act of 1996 (HIPAA) restricts the manner in which the Fund can divulge your protected health information (PHI).  If you wish to continue receiving verbal information from the Fund about your Fund records, you must acquire a PIN (personal identification number) from the Fund.  Without a pin number, the only way that you will be able to access your Fund information will be by making your request in writing.  Those needing pin numbers are; yourself, your spouse and any covered dependent that has reached the age of 18.

You can obtain pin numbers for yourself  and other members of your family by telephoning the Fund’s office at (631) 319-4099, or by sending in a request in writing.  You will need to provide your name, social security number, and the names of the people in your family who are also requesting a pin number.

If you would like to have someone other than yourself make inquiries on your behalf, you should also request an authorization form.  Once the authorization form is on file with the Fund, your designated representative will be given complete access to the information contained in your personal files until you choose to revoke your authorization.
